I Choose Me Counseling provides Case Management for people who need help handling the practical side of care. That may include referrals, outside services, school issues, work problems, family needs, housing concerns, or other barriers that make it harder to stay on track.

This service is there when therapy alone is not enough to hold everything together. Case management helps people keep up with next steps, stay connected to support, and deal with the parts of daily life that can interfere with progress.

What is Mental Health Targeted Case Management?

Skills Training

Real-time guidance on managing mental health symptomps and social stressors in the home, school and community.

Medication Support

Hands-on training to improve treatment compliance and understanding of medication regimens.

Wraparound Advocacy

Coordinating betwen schools, physicians and the justice system so no one falls through the cracks.

Medicaid-funded

A TMHP-sponsored program - no cost barrier for individuals with active Texas Medicaid coverage.

Who Case Management Is For

Case management is for people dealing with more than one problem at a time outside counseling. That might mean school issues, problems at home, work demands, family responsibilities, or support coming from several places at once.

It also serves parents and caregivers who are trying to keep track of contacts, forms, decisions, and deadlines for a child or family member. When support involves more than one provider, agency, or system, case management helps keep it organized.

How to make referral?

  1. Confirm the active TMHP/Texas coverage, individuals has Medicaid
  2. Verify they reside in a covered country (Galveston, Harris, Brazoria, Liberty or Chambers)
  3. Call or email our office with the Medicaid ID and parent/guardian contact info
  4. Our intake team will verify eligibility and schedule an on-site assessment within 24-48 hours
